In order for your application to move forward it is required to submit the following documents in addition to filling out the application: Driver’s Licence (front and back), Social Insurance Number, Health Card (front and back), Birth Certificate (front and back), School Transcripts (not diplomas – from all secondary and post-secondary education), Status Card (if applicable), First Aid/CPR Certificate (Level C preferred), O.A.C.P. Certificate of Results (COR) – with all valid components, and the Ontario Constable Selection System Consent and Release of Liability Form.
